Sagewood Middle School

Procedure for enrolling a student:

Contact the registrar or secretary to schedule an appointment with a guidance counselor. Appointments are scheduled between 7:30 am and 1:00 pm. Appointments may take 1 1/2 - 2 hours.

At the time of the appointment:

the student will take a math placement exam
parent will complete enrollment paperwork

parent must bring to appointment:

withdrawal form & address of last school attended

birth certificate

immunization records
proof of residency (one of the following):
- warranty deed/deed of trust
- lease agreement
- tax notice
- notarized letter, if living with a resident of Douglas County
Note: All students entering middle school must provide documentation of having completed the Hepatitis B series and two MMR vaccinations.
