Athletic Forms

The following forms and monies must be turned in together before player can practice during the season. Athletic fees are separate from other school fees and are collected per season. Fall sport registrations can be turned in on the days of school check-in.

A current Interscholastic Participation & Training Rules and DCSD Athletic/ACtivity Medical Info(good for one calendar year) is required, releasing the student to play sports. Plan ahead for physical appointments.

Athletic Emergency Information Card. Must fill out one entire form for each sport. (One copy stays in the office and one goes to the coach. So, we need both the top and bottom even though they are the same information.)

Participation fee of $100 per sport, not to exceed $375 per family, per school year.
