ImPACT Testing
General
Information
Please note – Students must be picked up or walk home. by
4:50 p.m. Monday-Thursday and 4:20 p.m. Fridays.
Transportation Responsibility
Students must be picked up within 15 minutes of the end of practice/game.
The following forms are due before an athlete can practice or
compete in a sport:
1. Interscholastic Participant Form: The Physician Permit is valid
for 365 days unless rescinded by the physician. A physician must
sign the permit.
2. Athletic Registration/Emergency Information Card: Given to
coaches as parent contact
information in case of injury at away competitions. A new card
is needed each year.
3. Registration Fee: $50.00 per sport not to exceed $125.00 per
athlete per school year.
4. Respect pledge: Athletes and parent must sign this good sportsmanship
pledge.

All forms and fees may be submitted to Ms. Dunn in the main office
prior to the start of each sport.

Practice and Games
• Practices will run Monday-Thursday from 3:05-4:50 p.m.
and Fridays from 3:05-4:20 p.m.
• Athletes cannot practice until all paperwork and fees
are completed and returned to the main office.
• If an athlete misses practice for any reason he/she must
notify the coach in writing.
• If an athlete is not in attendance during the day, they
will not be permitted to attend or participate in after school
activities for that day. Athletes must be at school at least half
of the day (4 hours) to attend or participate in activities. If
an athlete goes home ill during the day, they are not allowed
to return for athletics or activities that day.
Eligibility
Eligibility status will be determined on a student’s work
habits grade weekly. Students with one or more “Unsatisfactory”
grades will not be allowed to participate in one game/meet for
that week, but are still expected to practice.
